Programs for youth and young adults with developmental disabilities.

As an organization dedicated to providing teens and young adults who experience developmental disabilities the opportunity to lead a meaningful and inclusive life, our four core areas of support include: Independent Living Skill Development, Community Engagement Services, Youth Transition Services, and Supported Employment.

Through effective and person centered facilitation of these programs we provide those we serve with opportunity to increase their personal independence in their home and in their community, engage in inclusive social and leisure activities, prepare for transitioning from school based services into adulthood, as well as obtain and maintain employment in their community. 

Our programs are designed to be provided in tandem of one another to ensure those we support have the opportunity to live, work, and recreate in their communities and lead a self-directed life while still receiving the person-centered supports they need to reach their fullest potential.

Our services are provided in both the person's home and community, as well as employment settings. Programming is individualized and developed in partnership with the person receiving supports, and is customized to address the continuum of needs experienced by the youth and young adults we serve as well as their families. 

Summit Future Foundation Programs

Individualized Supports

Our Individualized Supports program assists an individual in increasing their personal independence for daily routines, both in their home and in the community.

Personal goals achieved through this program include:

  • Improvement of personal hygiene and self care activities

  • Increased independence in home management skills such as: maintaining personal living areas, laundry, and incidental household cleaning

  • Simple meal preparation and weekly meal planning 

  • Budgeting and management of personal finances

  • Accessing community through public transportation

Community Engagement Services

Summit Future Foundation’s Community Engagement Services provide opportunities for teens and adults who experience developmental disabilities to be actively engaged in their community, make friends and build personal relationships with others who have similar interests and ability levels, and develop their interpersonal skills. 

Each social group is uniquely designed to promote the productive use of leisure time throughout the community, and to facilitate growth in social, adaptive, and emotional skills.

Personal goals achieved through these programs include:

  • Enhanced understanding of what leisure opportunities are available to the person in their community

  • Increased ability to self select preferred community activities and to self direct involvement in community organizations and groups. 

  • Development of meaningful relationships and valued community roles

Watch Me Thrive Transition Services

Our signature transition planning program, Watch Me Thrive, focuses on supporting the individual served and their family in planning for life after school based services.

This innovative program takes a unique approach of supporting the participant in career planning and exploration, as well as developing employment readiness skills through work-based learning experiences while simultaneously supporting the family in creating a tactical transition plan that includes trajectory development, identifying ongoing support needs, and establishing an integrated support system. 

This program results in individuals and families who are better prepared for the transition from school based services into adulthood.

Supported Employment

Employment, and the opportunity to be engaged in work, can greatly enhance the quality of life of a person who experiences developmental disabilities.

Our Supported Employment programs are designed to provide those who have competitive and integrated vocation related goals the opportunity to obtain and maintain employment in their community by facilitating: exploration of viable career pathways and identification of local employers who provide employment opportunities that align with personal vocational goals, assistance with the application and onboarding process, on the job training in work related skills, and ongoing follow along support as needed to ensure ongoing success in the workplace.

Personal goals achieved through this program include:

  • Development of a personal career profile

  • Obtaining and maintaining competitive and integrated employment 

  • Increased independence in the workplace over time and through establishment of natural supports

Rights of the Persons We Serve: As a person-centered organization it is our policy to ensure that those we support are educated about their rights and the processes in which they may engage in if they feel their rights have been violated. It is the policy of the organization to ensure that all persons we serve have the right to safety, privacy, freedom of choice in the services they receive, where they receive them, and by whom they are provided. If you or your family member have engaged in our programs and services and you feel these rights have been violated, please submit a grievance form. We will review this information in a timely manner and work to address your concerns.
