Our Individualized Supports program assists those served in increasing their personal independence in daily routines, both in their home and in the community.
Information about Individualized Supports

Individuals who have a goal of increasing their independence in the home and in the community. Program participants must be 12 years of age or older, and demonstrate the need for skilled training in developing daily living skills.

In the person’s home and in community settings such as grocery stores, laundromats, the post office, and banks.

Our program implementation team strives to provide these services at a time that the routines being addressed would naturally occur. For example, if the person's goal is to be independent in morning routines, we would strive to have a community support professional in the home during the morning hours to assist with development in those skills. The frequency and duration of this programming is individually based on the needs and learning style of the persons served.

In collaboration with the person receiving services and others involved in supporting them, such as parents, guardians, and service coordinators. Goals and priorities are established for individual skill development in daily living routines.

Summit Future Foundation is a contracted provider through Missouri Department of Mental Health Division of Developmental Disabilities and our programs may be funded by Medicaid Waivers and/or Autism Project Funding.
